Active monitoring is built around 5 remotely delivered sessions of facilitated self-help, focused on early intervention with adult clients struggling with low to moderate severity mental health issues. Each client will have the option of one of seven pathways to focus on; ‘Anger’, ‘depression’, ‘grief and loss’, ‘anxiety and panic attacks’, ‘stress’, ‘self-esteem’ and ‘feeling alone’. Clients are given activities and strategies around their pathway of choice over the 6 weeks, as well as around other issues they may be facing, with guidance and support from a practitioner in each 20 minute session.
Clients can self-refer to this project through the attached referral form. The age range is 18+.
